Trans-arterial chemoembolization (TACE) as practiced and studied at Ain Shams University refers to a locoregional therapy for hepatocellular carcinoma (HCC). The protocol involves two primary methods: conventional TACE (cTACE) and drug-eluting bead TACE (DEB-TACE). cTACE utilizes an emulsion of the cytotoxic agent doxorubicin with lipiodol (an ethiodized oil) and urografin, followed by embolization with gel foam pledges to induce ischemia. DEB-TACE employs microspheres (DC beads) pre-loaded with doxorubicin, which provide both vessel occlusion and a sustained, controlled release of the chemotherapy directly into the tumor bed. The therapeutic effect is achieved through the dual action of doxorubicin-mediated DNA damage (via intercalation and topoisomerase II inhibition) and the deprivation of the tumor's blood supply, leading to coagulative necrosis.
