Transforming growth factor beta 1 (TGF-β1) mesenchymal stem cell-derived exosomes are engineered extracellular vesicles harvested from mesenchymal stem cells (MSCs) that have been genetically modified to overexpress the anti-inflammatory cytokine TGF-β1. These exosomes function as a cell-free therapeutic platform, combining the innate immunomodulatory properties of MSC-derived vesicles with the potent signaling of TGF-β1. In preclinical models of sepsis, these exosomes have demonstrated the ability to protect against systemic inflammation and organ failure by promoting the polarization of macrophages from a pro-inflammatory M1 phenotype to an anti-inflammatory M2 phenotype. This shift in macrophage activity helps resolve the cytokine storm and improve survival outcomes in inflammatory disease states.
