This is a combination regimen comprising four agents: trastuzumab, capecitabine, oxaliplatin, and pembrolizumab. - **Trastuzumab** is a monoclonal antibody targeting the human epidermal growth factor receptor 2 (HER2) on cancer cells. By binding to HER2, trastuzumab inhibits cell proliferation and induces immune-mediated cell death, particularly in HER2-positive tumors. - **Capecitabine** is an oral prodrug of 5-fluorouracil (5-FU), a chemotherapy agent that inhibits thymidylate synthase, thereby interfering with DNA synthesis in rapidly dividing cancer cells. - **Oxaliplatin** is a platinum-based chemotherapy that causes DNA crosslinking and apoptosis in cancer cells. - **Pembrolizumab** is a monoclonal antibody that blocks the programmed cell death protein 1 (PD-1) immune checkpoint on T cells, enhancing anti-tumor immune responses. This combination regimen brings together targeted therapy (trastuzumab), cytotoxic chemotherapy (capecitabine, oxaliplatin), and immune checkpoint blockade (pembrolizumab). It is being studied primarily in the treatment of HER2-positive gastric and gastroesophageal junction cancers and may be explored in other cancer types with similar molecular profiles. Capecitabine and oxaliplatin together are known as XELOX or CAPOX. Trastuzumab and XELOX have documented efficacy for HER2-positive advanced gastric cancer, and pembrolizumab is approved as part of first-line therapy in combination with trastuzumab, fluoropyrimidine, and platinum chemotherapy for HER2-positive advanced gastric or gastroesophageal junction adenocarcinoma with PD-L1 positivity[1][2][3][5].
