Trastuzumab + cetuximab beta + irinotecan is an investigational combination therapeutic regimen being evaluated for the treatment of HER2-positive metastatic colorectal cancer (mCRC). This triple combination therapy utilizes two monoclonal antibodies—trastuzumab, which targets the human epidermal growth factor receptor 2 (HER2), and cetuximab beta, which targets the epidermal growth factor receptor (EGFR)—alongside the cytotoxic chemotherapy agent irinotecan, a topoisomerase I inhibitor. The rationale for this combination is to overcome resistance to HER2-targeted therapies, which is often mediated by the constitutive activation of the EGFR pathway and the formation of EGFR-HER2 heterodimers. Sun Yat-sen University is the primary sponsor of a Phase II clinical trial investigating this regimen in patients with chemotherapy-refractory HER2-positive mCRC to determine if dual inhibition of the HER family receptors can enhance the efficacy of irinotecan.
