This combination therapy consists of the HER2-targeted monoclonal antibody trastuzumab and the chemotherapy regimen known as TEX, which includes docetaxel, oxaliplatin, and capecitabine. It was investigated in a Phase 1 dose-finding study (ECV trial) led by Odense University Hospital as a first-line treatment for patients with HER2-positive advanced gastro-esophageal adenocarcinoma, including cancers of the esophagus, cardia, and stomach. Trastuzumab works by binding to the human epidermal growth factor receptor 2 (HER2), inhibiting tumor growth signaling and inducing antibody-dependent cellular cytotoxicity (ADCC). The chemotherapy components provide synergistic cytotoxic effects: docetaxel stabilizes microtubules to prevent cell division, oxaliplatin induces DNA cross-linking to inhibit replication, and capecitabine acts as a prodrug of 5-fluorouracil to inhibit thymidylate synthase and disrupt DNA synthesis.
