Overview

A combination therapy consisting of trastuzumab, ramucirumab, and paclitaxel used primarily as a second-line treatment for HER2-positive advanced gastric or gastroesophageal junction (G/GEJ) cancer. This regimen works through multiple mechanisms: trastuzumab targets HER2, ramucirumab inhibits VEGFR2 (blocking angiogenesis), and paclitaxel disrupts microtubule function. The simultaneous inhibition of HER2 signaling pathways and angiogenesis appears to improve patient outcomes compared to standard second-line treatment with ramucirumab and paclitaxel alone. The regimen is administered in a 28-day cycle with trastuzumab (4mg/kg loading dose followed by 2mg/kg weekly), ramucirumab (8mg/kg on days 1 and 15), and paclitaxel (80mg/m² on days 1, 8, and 15). This combination has shown noteworthy antitumor activity with manageable safety profiles in clinical trials, particularly the HER-RAM study (NCT04888663).
