Trastuzumab deruxtecan + afatinib is an investigational combination therapy being evaluated for the treatment of HER2-low advanced or metastatic gastric cancer and gastroesophageal junction (GEJ) adenocarcinoma. This regimen combines trastuzumab deruxtecan (T-DXd), an antibody-drug conjugate (ADC) targeting HER2 with a topoisomerase I inhibitor payload, and afatinib, an irreversible tyrosine kinase inhibitor (TKI) of the ErbB receptor family (EGFR, HER2, and ErbB4). The combination is the focus of the VIKTORY-2 clinical trial, led by researcher Jeeyun Lee, which aims to investigate whether dual blockade of the HER2 pathway can improve clinical outcomes in patients with low HER2 expression who have progressed on prior therapies. T-DXd is developed by Daiichi Sankyo and AstraZeneca, while afatinib is a product of Boehringer Ingelheim.
