Trastuzumab deruxtecan + capecitabine + bevacizumab is an investigational combination therapy regimen being evaluated for the treatment of HER2-positive metastatic or locally advanced unresectable colorectal cancer. This triplet therapy combines three distinct mechanisms: trastuzumab deruxtecan (T-DXd), a HER2-directed antibody-drug conjugate that delivers a cytotoxic topoisomerase I inhibitor payload directly to HER2-expressing cells; capecitabine, an oral fluoropyrimidine carbamate that is enzymatically converted to 5-fluorouracil to inhibit DNA synthesis; and bevacizumab, a monoclonal antibody that inhibits vascular endothelial growth factor (VEGF) to suppress tumor angiogenesis. The regimen is the focus of the Phase II CHIMERA study, led by the Gruppo Oncologico del Nord-Ovest (GONO), aimed at improving outcomes in patients with HER2-amplified colorectal cancer who have limited first-line options.
