Overview

Trastuzumab deruxtecan + ivonescimab is an investigational combination therapy comprising a HER2-directed antibody-drug conjugate (ADC) and a bispecific antibody. Trastuzumab deruxtecan (T-DXd) consists of a humanized anti-HER2 monoclonal antibody linked to a topoisomerase I inhibitor payload (deruxtecan), designed to target and kill HER2-expressing tumor cells. Ivonescimab is a first-in-class bispecific antibody that simultaneously targets programmed cell death protein 1 (PD-1) and vascular endothelial growth factor (VEGF), combining immunotherapy with anti-angiogenesis. This combination is being evaluated in Phase II clinical trials (e.g., the UNITY study) for patients with hormone receptor-positive (HR+), HER2-low or HER2-ultralow advanced breast cancer, including those with central nervous system (CNS) metastases.
