Overview

This is a combination therapy comprising three drugs: **trastuzumab deruxtecan** (an antibody-drug conjugate targeting HER2), **volrustomig** (a bispecific monoclonal antibody targeting PD-1 and TIGIT, also known in some studies and early communications as AZD0486), and **fluorouracil** (a classic cytotoxic antimetabolite, also known as 5-FU). The combination is under clinical investigation as a first-line treatment for HER2-positive metastatic or locally advanced gastric and gastroesophageal junction adenocarcinoma. Trastuzumab deruxtecan delivers a cytotoxic payload specifically to HER2-expressing tumor cells, while volrustomig potentiates anti-tumor immunity by blocking both PD-1 and TIGIT immune checkpoints, and fluorouracil disrupts DNA synthesis in dividing cells. The combination aims to synergistically increase anti-tumor efficacy by integrating targeted therapy, immunotherapy, and chemotherapy modalities[5][9].
