Trastuzumab rezetecan + toripalimab is an investigational combination therapy being evaluated for the neoadjuvant treatment of hormone receptor-positive (HR+), HER2-low breast cancer. The combination consists of trastuzumab rezetecan (SHR-A1811), a HER2-directed antibody-drug conjugate (ADC) developed by Jiangsu HengRui Medicine, and toripalimab, an anti-PD-1 monoclonal antibody developed by Shanghai Junshi Biosciences. Trastuzumab rezetecan consists of a humanized anti-HER2 antibody conjugated to a DNA topoisomerase I inhibitor payload via a cleavable tetrapeptide-based linker, designed to deliver potent cytotoxicity to HER2-expressing tumor cells. Toripalimab is an immune checkpoint inhibitor that enhances the body's anti-tumor immune response by blocking the PD-1/PD-L1 pathway. This combination approach aims to leverage the synergistic effects of targeted chemotherapy and immunotherapy in the neoadjuvant setting, specifically targeting the HR+/HER2-low breast cancer subtype to improve pathologic complete response (pCR) rates.
