Overview

TRBV9xCD3 bispecific antibody is an experimental precision immunotherapy designed to selectively deplete autoreactive T cells expressing the T cell receptor beta-chain variable gene TRBV9. Developed by researchers at Johns Hopkins University and associated with TBD Pharma, this bispecific T-cell engager (BiTE) or single-chain diabody (scDb) redirects CD3+ effector T cells to eliminate TRBV9+ T cells. This approach aims to treat autoimmune conditions like HLA-B27+ axial spondyloarthritis (ankylosing spondylitis) and HLA-DQ8+ celiac disease, where the autoreactive T cell compartment is highly skewed toward TRBV9 usage, while sparing over 95% of the total T cell repertoire to minimize generalized immunosuppression. Preclinical studies presented at ACR Convergence 2024 and EULAR 2025 have demonstrated the antibody's ability to selectively eliminate disease-relevant T cell clonotypes in patient-derived samples.
