Treated Dentin Matrix (TDM) is a bioactive, acellular scaffold derived from human dentin, developed for regenerative endodontic procedures such as pulpotomy and pulp capping. It is produced by demineralizing extracted human teeth to remove cellular components while preserving the natural extracellular matrix and endogenous bioactive proteins, including transforming growth factor-beta 1 (TGF-β1), bone morphogenetic proteins (BMPs), and fibroblast growth factors (FGFs). When applied to exposed dental pulp, TDM acts as a biological template that recruits dental pulp stem cells (DPSCs) and induces their differentiation into odontoblast-like cells, which then secrete reparative dentin. This process facilitates the formation of a dentin bridge and supports continued root development (apexogenesis) in immature permanent teeth. TDM is currently being evaluated in Phase 3 clinical trials at Ain Shams University as a biomimetic alternative to traditional materials like Mineral Trioxide Aggregate (MTA) and bioceramics for treating deep dental caries and pulp exposure.
