Treg-depleted donor lymphocyte infusion (DLI) is an adoptive cellular immunotherapy developed by Assistance Publique - Hôpitaux de Paris for the treatment of relapsed hematological malignancies following allogeneic hematopoietic stem cell transplantation (HSCT). The therapy involves the infusion of donor lymphocytes from which regulatory T cells (Tregs) have been selectively removed. Tregs are known to suppress anti-tumor immune responses; by depleting them, the graft-versus-leukemia (GVL) effect of the donor T cells is enhanced. While earlier strategies targeted CD25 for depletion, this specific approach utilizes CD127 (interleukin-7 receptor alpha) positive selection to more effectively isolate and remove the Treg population, thereby amplifying the allogeneic immune response against malignant cells.
