Tregexo is an experimental exosome-based therapeutic consisting of extracellular vesicles derived from regulatory T cells (Tregs). It is currently being evaluated in clinical trials for the treatment of refractory neovascular (wet) age-related macular degeneration (nAMD), often in combination with anti-VEGF agents like ranibizumab. The therapeutic rationale for Tregexo is based on the natural immunosuppressive and anti-inflammatory properties of Treg-derived exosomes, which carry bioactive molecules capable of modulating the ocular microenvironment. By addressing the inflammatory components of AMD that may contribute to anti-VEGF resistance, Tregexo aims to improve visual outcomes in patients who do not respond adequately to standard monotherapy.
