Overview

Triadcortyl (also known as Tri-Adcortyl) is a combination topical medication containing four active ingredients: the corticosteroid triamcinolone acetonide, the aminoglycoside antibiotic neomycin sulfate, the polypeptide antibiotic gramicidin, and the polyene antifungal nystatin. It is indicated for the treatment of inflammatory skin conditions (such as eczema and dermatitis) and otitis externa where secondary bacterial or fungal infection is present or suspected. Triamcinolone acetonide provides anti-inflammatory and antipruritic effects by acting as an agonist at the glucocorticoid receptor. Neomycin and gramicidin provide broad-spectrum antibacterial coverage; neomycin inhibits protein synthesis by binding to the 30S ribosomal subunit, while gramicidin disrupts bacterial cell membrane integrity by forming cation-selective channels. Nystatin targets fungal pathogens by binding to ergosterol in the cell membrane, leading to increased permeability and cell death. Developed and marketed by Bristol Myers Squibb, the product has been discontinued in several markets, including the United Kingdom.
