Overview

triamcinolone acetonide + bupivacaine + epinephrine is a combination drug therapy being investigated by Eastern Virginia Medical School for the treatment of chronic neurogenic cough. The treatment involves a superior laryngeal nerve (SLN) block using a mixture of the corticosteroid triamcinolone acetonide and the local anesthetic bupivacaine with epinephrine. Triamcinolone acts as a glucocorticoid receptor agonist to provide long-term anti-inflammatory effects and reduce neural hypersensitivity, while bupivacaine acts as a sodium channel blocker to provide immediate neural blockade. Epinephrine is included as a vasoconstrictor to prolong the local effect of the anesthetic. This intervention is designed to interrupt the cough reflex arc in patients whose cough is thought to have a neurogenic origin and has failed standard therapies.
