Overview

This combination therapy is a specialized solution administered via ductal lavage for the treatment of idiopathic granulomatous mastitis (IGM). Developed by Sun Yat-Sen Memorial Hospital of Sun Yat-Sen University, the regimen consists of the synthetic corticosteroid triamcinolone acetonide, the third-generation cephalosporin antibiotic ceftriaxone, and the local anesthetic lidocaine, prepared in a saline vehicle. The treatment involves the intraductal infusion of approximately 25 mL of this solution (containing 40 mg triamcinolone acetonide, 1.0 g ceftriaxone, and 5 mL of 2% lidocaine) directly into the lactiferous ducts over a 20-25 minute period. This localized delivery method is designed to provide high concentrations of anti-inflammatory and antimicrobial agents to the site of disease while minimizing the systemic side effects typically associated with long-term oral corticosteroid use. It is currently being evaluated in a multicenter, randomized, open-label Phase 3 trial (NCT03724903) as a non-inferior alternative to standard oral prednisone or methylprednisolone therapy.
