Triamcinolone acetonide + ziv-aflibercept is a combination therapy used off-label for the treatment of central diabetic macular edema (DME). Triamcinolone acetonide is a synthetic corticosteroid with potent anti-inflammatory and immunosuppressive properties, commonly administered via suprachoroidal injection to reduce retinal inflammation and vascular permeability. Ziv-aflibercept is a recombinant fusion protein that acts as a soluble decoy receptor for vascular endothelial growth factor (VEGF)-A, VEGF-B, and placental growth factor (PIGF), thereby inhibiting abnormal blood vessel growth and leakage in the retina. Ziv-aflibercept has an identical structure to aflibercept but differs in formulation characteristics such as osmolarity; it is approved for intravenous use in metastatic colorectal cancer but has been studied off-label via intravitreal injection for ocular diseases due to its lower cost compared to standard aflibercept. The combination aims to leverage both anti-VEGF and corticosteroid mechanisms to improve visual acuity and reduce macular thickness in DME patients[6][1][7].
