Overview

Triamterene is a potassium-sparing diuretic used to treat edema (fluid retention) associated with conditions such as congestive heart failure, liver cirrhosis, nephrotic syndrome, and secondary hyperaldosteronism. It is also used alone or in combination with other medications to manage hypertension (high blood pressure). Triamterene works by directly inhibiting the epithelial sodium channel (ENaC) in the distal renal tubule and collecting duct of the nephron. This action blocks sodium reabsorption while reducing potassium excretion, thereby promoting diuresis without significant loss of potassium. Unlike spironolactone, which antagonizes aldosterone receptors, triamterene acts directly on ENaC channels. The drug is often combined with thiazide diuretics to enhance efficacy and maintain potassium balance.
