Overview

Tricaprilin is an investigational, orally administered medium-chain triglyceride (MCT) composed of three eight-carbon fatty acids (caprylic acid). Developed by Cerecin (formerly Accera), it is designed to address cerebral glucose hypometabolism, a common pathophysiological feature of Alzheimer's disease and other neurological disorders. Upon ingestion, tricaprilin is hydrolyzed by gastrointestinal lipases into octanoic acid, which is then metabolized by the liver into the ketone bodies acetoacetate and β-hydroxybutyrate. These ketones cross the blood-brain barrier to serve as an alternative energy source for neuronal mitochondria, bypassing impaired glucose transport and metabolism. The compound has been evaluated in Phase 3 clinical trials for Alzheimer's disease and has shown potential in pilot studies for migraine prevention and infantile spasms.
