This combination regimen is a chemo-immunotherapy approach being evaluated for the first-line treatment of advanced gastric and gastroesophageal junction (GEJ) adenocarcinoma. It consists of trifluridine/tipiracil (TAS-102), an oral nucleoside-based cytotoxic agent; oxaliplatin, a platinum-based chemotherapy; and sintilimab, a recombinant humanized anti-PD-1 monoclonal antibody. Trifluridine is phosphorylated into a cytotoxic form that incorporates into DNA, while tipiracil inhibits its degradation by thymidine phosphorylase, thereby increasing systemic exposure. Oxaliplatin induces DNA cross-linking, leading to apoptosis and synergistic cytotoxic effects. Sintilimab blocks the PD-1/PD-L1 pathway, reinvigorating the anti-tumor immune response. The regimen is primarily being studied in investigator-initiated trials led by Renji Hospital, Shanghai Jiao Tong University School of Medicine.
