Trihexyphenidyl is a centrally acting antimuscarinic (anticholinergic) small molecule used primarily to manage symptoms of Parkinson's disease, including stiffness, tremors, spasms, and poor muscle control. It is also indicated for the treatment and prevention of extrapyramidal symptoms caused by certain antipsychotic medications. The drug works by antagonizing muscarinic acetylcholine receptors in the central nervous system, thereby reducing cholinergic activity that contributes to motor dysfunction. While it can provide symptomatic relief—especially for tremor—it does not cure Parkinson’s disease and is generally considered adjunctive therapy due to its side effect profile. Other uses include management of dystonia in children with cerebral palsy, essential tremor, akathisia, and as an antidote in organophosphate poisoning[1][2][4][5][6].
