Triiodothyronine sulfate (T3S) is a small molecule prodrug and endogenous metabolite of the thyroid hormone triiodothyronine (T3), developed by Bracco for the treatment of hypothyroidism. When administered orally, T3S is converted to active T3 in a dose-dependent manner by sulfatases in peripheral tissues, producing steady-state serum T3 concentrations for up to 48 hours. This pharmacokinetic profile offers a significant advantage over conventional liothyronine (T3) supplements, which often cause supra-physiological peaks and rapid declines in hormone levels. T3S has been investigated in Phase II clinical trials as a partial replacement for levothyroxine (L-T4) in thyroidectomized patients to achieve more physiological thyroid hormone replacement and improve metabolic control.
