Trilaciclib + pemetrexed is an investigational combination therapy consisting of trilaciclib, a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor, and pemetrexed, a folate antimetabolite. Trilaciclib is designed to protect bone marrow from chemotherapy-induced damage by transiently arresting hematopoietic stem and progenitor cells in the G1 phase of the cell cycle. Pemetrexed inhibits several enzymes involved in folate metabolism and DNA synthesis, including thymidylate synthase. The combination is being studied primarily for its potential to improve outcomes in patients with non-small cell lung cancer (NSCLC), particularly those with leptomeningeal metastasis or lung adenocarcinoma[3][4]. Trilaciclib has been approved as Cosela for reducing chemotherapy-induced myelosuppression in small cell lung cancer but remains investigational for use with pemetrexed outside clinical trials[7].
