Overview

TRIO-863 is a first-in-class bispecific antibody targeting CD33 and TRAIL-R2 (Death Receptor 5) for the treatment of relapsed or refractory acute myeloid leukemia (AML). Developed by Trio Pharmaceuticals in collaboration with academic institutions including Purdue University and MD Anderson Cancer Center, TRIO-863 is designed to selectively induce apoptosis in AML blasts by dually engaging CD33 and TRAIL-R2. The antibody binds CD33 with high affinity, which serves to localize and restrict the activation of TRAIL-R2 to CD33-positive leukemic cells, thereby minimizing off-target effects on normal hematopoietic stem cells and liver tissue. Preclinical data presented at AACR 2026 demonstrated that TRIO-863 is highly potent against high-risk TP53-mutant and drug-resistant AML models, showing significantly greater anti-leukemic activity than gemtuzumab ozogamicin while sparing normal progenitor cells.
