Overview

Triple Intrathecal Chemotherapy is a combination regimen in which three drugs—methotrexate, cytarabine (also known as cytosine arabinoside), and hydrocortisone—are administered directly into the cerebrospinal fluid (CSF) via lumbar puncture or an Ommaya reservoir. This approach is used for both treatment and prophylaxis of central nervous system (CNS) involvement in cancers such as acute lymphoblastic leukemia (ALL), lymphoma, and leptomeningeal carcinomatosis from solid tumors. Methotrexate and cytarabine are antimetabolite chemotherapeutic agents that inhibit DNA synthesis in rapidly dividing cells, while hydrocortisone is a corticosteroid added to reduce inflammation and lower the risk of chemical meningitis. The combination aims to maximize anti-cancer efficacy within the CNS compartment where systemic chemotherapy penetration is limited[1][2][5][8].
