Trivalent oral poliovirus vaccine (tOPV) is a live attenuated viral vaccine containing Sabin strains of poliovirus types 1, 2, and 3. Administered as oral drops, the vaccine viruses replicate in the intestinal mucosa, inducing robust local secretory IgA and systemic IgG antibodies. This dual immune response provides comprehensive protection against paralytic poliomyelitis caused by all three wild poliovirus serotypes. Historically, tOPV was the primary tool for the Global Polio Eradication Initiative; however, following the eradication of wild poliovirus type 2, global health programs transitioned to bivalent OPV (types 1 and 3) in 2016 to minimize the risk of vaccine-derived poliovirus type 2 outbreaks.
