Overview
tropicamide + phenylephrine + lidocaine + diclofenac is an ophthalmic fixed-combination concept intended to provide rapid pupil dilation, ocular anesthesia, and perioperative anti-inflammatory activity in the setting of ophthalmic procedures, especially cataract surgery. Tropicamide is a short-acting antimuscarinic mydriatic that blocks muscarinic receptors in the iris sphincter and ciliary muscle; phenylephrine is an alpha-1 adrenergic agonist that contracts the iris dilator muscle to augment mydriasis; lidocaine is an amide local anesthetic that blocks voltage-gated sodium channels to produce local ocular anesthesia; and diclofenac is a nonsteroidal anti-inflammatory drug that inhibits cyclooxygenase enzymes and reduces prostaglandin-mediated intraoperative miosis, pain, and inflammation. The exact four-ingredient product does not appear to correspond to a clearly established marketed branded medicine identified from standard regulatory sources, but the combination is pharmacologically coherent and relates to published ophthalmic dilation regimens and to marketed three-drug intracameral products in the same therapeutic area.
