This is a combination therapy consisting of the cancer vaccine TroVax (MVA-5T4), fluorouracil (a pyrimidine analog antimetabolite), irinotecan (a topoisomerase I inhibitor), and leukovorin (folinic acid). TroVax is a therapeutic cancer vaccine based on a highly attenuated strain of vaccinia virus (modified vaccinia Ankara) engineered to express the human oncofetal antigen 5T4. The mechanism of action involves stimulating an immune response against tumor cells expressing the 5T4 antigen—a transmembrane glycoprotein found at high levels in many solid tumors but rarely in normal tissues. Fluorouracil inhibits thymidylate synthase and disrupts DNA synthesis; irinotecan inhibits topoisomerase I leading to DNA damage; and leukovorin enhances the cytotoxic effects of fluorouracil by stabilizing its binding to thymidylate synthase. This combination aims to leverage both immunotherapeutic and cytotoxic mechanisms for enhanced anti-tumor efficacy[3][4][1][2].
