Tuberculin purified protein derivative (PPD) is a diagnostic biologic used to identify individuals infected with *Mycobacterium tuberculosis*. It consists of a sterile, purified fraction of the soluble growth products of the tubercle bacillus. When administered intradermally (the Mantoux test), it serves as a recall antigen that triggers a Type IV delayed-type hypersensitivity (DTH) reaction in persons with prior exposure to TB. This reaction, characterized by localized induration and erythema, is mediated by sensitized T-lymphocytes that migrate to the injection site and release pro-inflammatory cytokines. Beyond its primary diagnostic role, PPD is being investigated as an intralesional immunotherapeutic agent for conditions like anogenital warts and certain cancers, leveraging its ability to stimulate a potent Th1-biased immune response.
