Tucatinib + trastuzumab + pertuzumab is an investigational combination therapy for the maintenance treatment of patients with unresectable locally advanced or metastatic HER2-positive breast cancer whose disease has not progressed on induction chemotherapy with trastuzumab, pertuzumab, and a taxane. Tucatinib is a highly selective, reversible tyrosine kinase inhibitor targeting the HER2 receptor, which inhibits downstream signaling pathways involved in cell proliferation and survival. Trastuzumab and pertuzumab are monoclonal antibodies that bind to different epitopes of the HER2 receptor; trastuzumab blocks ligand-independent HER2 signaling and mediates antibody-dependent cellular cytotoxicity, while pertuzumab prevents dimerization of HER2 with other members of the ErbB family. The combination aims to provide more comprehensive blockade of HER2-driven tumor growth. This regimen is currently being evaluated in phase 3 clinical trials as maintenance therapy following standard induction treatment[5][6][7][10].
