Tucidinostat + azacitidine + cyclophosphamide + epirubicin + vindesine + prednisone (also referred to as AC-CHOP in clinical trial contexts) is an experimental combination regimen being evaluated for the first-line treatment of peripheral T-cell lymphoma (PTCL). The regimen combines two epigenetic modulators—tucidinostat (a subtype-selective HDAC inhibitor targeting HDAC1, 2, 3, and 10) and azacitidine (a DNA methyltransferase inhibitor)—with a modified CHOP chemotherapy backbone. This approach is based on the premise that epigenetic priming with HDAC inhibitors and hypomethylating agents can sensitize T-cell lymphoma cells to conventional cytotoxic chemotherapy. Peking Union Medical College Hospital is the lead sponsor for a Phase III trial investigating this combination's efficacy and safety compared to standard CHOP in untreated patients.
