The tumor blood vessel antigen peptide-pulsed alpha-type-1 polarized dendritic cell vaccine (DC-TBVA) is an experimental autologous, personalized cancer immunotherapy. It is manufactured by isolating a patient's own peripheral blood mononuclear cells, generating alpha-type-1 polarized dendritic cells (alphaDC1s) ex vivo, and pulsing them with six HLA-A2-restricted tumor blood vessel antigen (TBVA)-derived peptides: DLK1, EphA2, HBB, NRP1, RGS5, and TEM1. Unlike traditional cancer vaccines that target tumor cells directly, this vaccine targets the tumor stroma, specifically HLA-A2+ pericytes and vascular endothelial cells, with the goal of inducing CD8+ T cell-mediated immunity to disrupt the tumor microenvironment and blood supply. It has been investigated in Phase 1 clinical trials for metastatic breast cancer, often in combination with gemcitabine to suppress myeloid-derived suppressor cells (MDSCs) and regulatory T cells (Tregs).
