Overview

Tumor-draining lymph node (TDLN) lymphocyte therapy is an autologous adoptive cell therapy developed by The Cleveland Clinic for the treatment of high-grade gliomas, including glioblastoma multiforme and anaplastic astrocytoma. The therapeutic process begins with the sensitization of the patient's immune system in vivo through the administration of an autologous tumor cell vaccine, often combined with sargramostim (GM-CSF). Following this priming, lymphocytes are surgically harvested from the lymph nodes draining the tumor site. These sensitized T cells are then expanded and activated ex vivo using mitogens and cytokines such as Interleukin-2 (IL-2). The resulting population of tumor-specific effector T cells is re-infused into the patient, typically following lymphodepleting chemotherapy with cyclophosphamide, to target and eliminate residual glioma cells.
