This autologous tumor-infiltrating lymphocyte (TIL) therapy is a cellular immunotherapy developed by the H. Lee Moffitt Cancer Center and Research Institute for the treatment of metastatic, high-grade soft tissue sarcoma in adolescent and young adult patients. The therapy involves the extraction of naturally occurring T cells from a patient's resected tumor, which are then expanded ex vivo to clinically relevant numbers. The treatment protocol includes a lymphodepleting conditioning regimen of cyclophosphamide and fludarabine, followed by the infusion of the expanded TILs and subsequent administration of high-dose interleukin-2 (IL-2) to promote T-cell survival and activity. This adoptive cell transfer approach leverages the patient's own immune system to recognize and destroy cancer cells through T-cell receptor-mediated recognition of tumor-associated antigens, providing a personalized treatment option for patients who have failed standard therapies.
