Tusamitamab R848 ADC is an investigational immune-stimulating antibody-drug conjugate (ISAC) developed by Sanofi. It consists of the anti-CEACAM5 monoclonal antibody, tusamitamab, conjugated to resiquimod (R848), a dual Toll-like receptor 7 and 8 (TLR7/8) agonist. Unlike traditional ADCs that utilize cytotoxic payloads for direct cell killing, this ISAC is designed to activate the innate immune system within the tumor microenvironment. By targeting CEACAM5-expressing tumor cells, the conjugate localizes the TLR7/8 agonist to the tumor site, where it interacts with myeloid cells (such as monocytes and dendritic cells) via the antibody's Fc region. This interaction triggers myeloid cell activation, maturation, and phagocytosis, thereby promoting a robust anti-tumor immune response. Preclinical studies have demonstrated significant antitumor activity and complete regressions in CEACAM5-positive pancreatic and lung cancer models.
