TV-1380 is a recombinant fusion protein designed for the treatment of cocaine addiction and overdose. It consists of a rationally mutated human butyrylcholinesterase (BChE) enzyme fused to human serum albumin, which extends its half-life in circulation. The engineered BChE component has enhanced catalytic activity against cocaine, enabling rapid hydrolysis and elimination of cocaine from plasma before it can enter the brain or heart. This mechanism reduces both the psychoactive and toxic effects of cocaine. Preclinical studies demonstrated that TV-1380 protects against cocaine-induced toxicity and prevents relapse in animal models by accelerating peripheral degradation of cocaine. Clinical trials have shown that TV-1380 is safe, well tolerated, and exhibits dose-proportional pharmacokinetics with a long half-life (43–77 hours), supporting once-weekly dosing[1][2][6]. The drug was developed by Teva Pharmaceutical Industries. Its primary indication is for treating cocaine-related disorders by enhancing peripheral metabolism of cocaine to non-toxic metabolites[1][2][6].
