TW-012R is a small molecule drug candidate developed by Time Therapeutics for the treatment of Alzheimer's disease, specifically targeting patients with Presenilin 1 (PSEN1) mutations. Chemically, TW-012R is the (R)-enantiomer of ramelteon, a known melatonin receptor agonist. The drug's mechanism of action involves the activation of Melatonin receptor 1 (MT1) and Melatonin receptor 2 (MT2), which are critical components of the body's circadian rhythm regulation. Time Therapeutics' approach, termed the "Circadian Clock Hypothesis," posits that restoring normal circadian function can enhance the glymphatic clearance of amyloid-beta and reduce neuroinflammation, thereby slowing or reversing the progression of neurodegeneration. TW-012R is currently in early-stage clinical development, aiming to provide a targeted therapy for familial forms of Alzheimer's disease where circadian disruption is a prominent early feature.
