This approach involves a sequential two-step process: first, patients receive a fixed dose of donor CD3+ T cells following conditioning to provide a potent graft-versus-leukemia effect. This is followed by the administration of high-dose cyclophosphamide to induce T-cell tolerization and selectively deplete alloreactive T cells, thereby reducing the risk of graft-versus-host disease (GVHD). Finally, CD34-selected hematopoietic stem cells are infused to ensure rapid hematopoietic recovery. This protocol is particularly significant for its ability to facilitate successful transplantation using haploidentical (half-matched) related donors, significantly expanding the donor pool for patients who lack fully matched family members.
