Overview

The Two-Step Haploidentical Stem Cell Transplantation is a specialized allogeneic hematopoietic stem cell transplant (HSCT) regimen developed and evaluated by Northwell Health for the treatment of high-risk hematologic malignancies in pediatric and young adult patients. This protocol is designed for patients who lack a human leukocyte antigen (HLA)-matched donor and utilizes a haploidentical (half-matched) related donor. The methodology involves a sequential two-step process: first, the administration of a fixed dose of donor T-cells (Donor Lymphocyte Infusion) to promote early immune reconstitution and provide a graft-versus-leukemia effect, followed by high-dose cyclophosphamide for in vivo T-cell depletion to mitigate graft-versus-host disease (GVHD). The second step consists of the infusion of CD34-selected hematopoietic stem cells. This approach aims to balance the need for rapid immune recovery with the prevention of severe GVHD in the mismatched setting.
