Type 1 interferon aerosol is an investigational biologic therapy consisting of a type 1 interferon (such as interferon-alpha or interferon-beta) formulated for delivery via inhalation. Developed for the treatment of severe respiratory viral infections, including COVID-19, it functions by binding to the heterodimeric Type I interferon receptor (IFNAR1/IFNAR2) on the surface of target cells. This binding activates the JAK-STAT signaling pathway, leading to the transcription of numerous interferon-stimulated genes (ISGs) that inhibit viral entry, replication, and assembly. The aerosolized delivery method is designed to provide high concentrations of the antiviral cytokine directly to the primary site of infection in the lungs while potentially reducing the systemic toxicity associated with parenteral administration. Clinical evaluation, including studies by the Shanghai Public Health Clinical Center, has focused on its safety and efficacy as an adjunct to standard care in hospitalized patients.
