U-13C6-D-glucose is a stable isotope-labeled diagnostic tracer consisting of D-glucose in which all six carbon-12 atoms have been replaced by carbon-13 (13C). Developed for clinical research by the Erasmus Medical Center, it is utilized in breath testing to non-invasively detect mesenteric ischemia. The diagnostic principle relies on the fact that the absorption and subsequent aerobic metabolism of glucose by the intestinal mucosa are oxygen-dependent processes. Following oral administration, the labeled glucose is metabolized into 13CO2, which is then excreted in the breath. By measuring the ratio of 13CO2 to 12CO2 using mass spectrometry, clinicians can quantify mucosal oxygen consumption and distinguish between aerobic and anaerobic epithelial metabolism, thereby identifying regions of ischemia where oxygen supply is insufficient to support normal metabolic activity.
