Ulipristal is a synthetic selective progesterone receptor modulator (SPRM) used primarily as an emergency contraceptive to prevent pregnancy after unprotected intercourse or contraceptive failure. It acts mainly by inhibiting or delaying ovulation through antagonistic and partial agonist effects at the progesterone receptor, thereby suppressing the luteinizing hormone surge necessary for follicular rupture. Ulipristal may also alter endometrial thickness, potentially affecting implantation, though its primary action is pre-fertilization. It is a derivative of 19-norprogesterone and has some affinity for glucocorticoid receptors but with lower activity than mifepristone.
