Overview

Umbilical cord blood is a biologic product containing hematopoietic stem and progenitor cells (HSPCs) collected from the placenta and umbilical cord following childbirth. It is used as a source of stem cells for hematopoietic stem cell transplantation (HSCT) in patients with hematological malignancies, bone marrow failure syndromes, and certain genetic disorders. The primary therapeutic mechanism involves the reconstitution of the recipient's hematopoietic and immune systems following myeloablative or non-myeloablative conditioning. Cord blood is particularly valuable as an alternative to bone marrow or peripheral blood stem cells due to its lower requirement for HLA matching, although it is often associated with a lower total cell dose and slower engraftment. Several cord blood products are FDA-approved for hematopoietic reconstitution in patients with disorders affecting the hematopoietic system.
