Overview

Umbilical cord blood + cytarabine is a combination therapeutic regimen involving the infusion of third-party umbilical cord blood (UCB) following cytarabine-based chemotherapy. Developed by the Hematology Department at The First Affiliated Hospital of Dalian Medical University, this approach is primarily investigated for the treatment of acute myeloid leukemia (AML), particularly in patients who are not candidates for standard allogeneic hematopoietic stem cell transplantation. Cytarabine, a pyrimidine analogue, acts as an antimetabolite that inhibits DNA polymerase and incorporates into DNA, leading to the death of rapidly dividing leukemic cells. The subsequent infusion of third-party UCB provides hematopoietic stem and progenitor cells intended to accelerate hematopoietic recovery and potentially induce a graft-versus-leukemia (GVL) effect, where the donor immune cells help eliminate residual malignant cells without the need for full donor-host matching or intensive conditioning.
