Umbilical cord blood cell therapy, as developed and studied at the Masonic Cancer Center, University of Minnesota, involves the transplantation of hematopoietic stem cells (HSCs) derived from unrelated donor umbilical cord blood units. This cellular therapy is utilized for the treatment of a broad spectrum of hematological malignancies, including various leukemias, lymphomas, and myelodysplastic syndromes, as well as bone marrow failure syndromes. The therapeutic process involves the infusion of cord blood cells—often using a double-unit graft to ensure adequate cell dose—following a conditioning regimen such as the non-myeloablative combination of cyclophosphamide, fludarabine, and total body irradiation (TBI). The mechanism of action is based on the engraftment of donor HSCs into the recipient's bone marrow, where they proliferate and differentiate to reconstitute a healthy hematopoietic system and provide a graft-versus-leukemia effect.
