Umbilical cord-derived mesenchymal stromal cell (UC-MSC) secretome is a cell-free therapeutic product consisting of the complete set of bioactive molecules secreted by UC-MSCs into their extracellular environment. This complex mixture includes cytokines, chemokines, growth factors, and extracellular vesicles (such as exosomes) that mediate the immunomodulatory, anti-inflammatory, and regenerative properties associated with MSC therapy. By utilizing the secretome rather than the cells themselves, this approach aims to provide the therapeutic benefits of MSCs while avoiding potential risks related to cell transplantation, such as immunogenicity or unwanted differentiation. In clinical development, particularly by Universitas Sriwijaya, it is being evaluated as an intramuscular adjunct therapy for moderate rheumatoid arthritis to reduce synovial inflammation and improve clinical outcomes in patients who are inadequately controlled by conventional synthetic disease-modifying antirheumatic drugs (csDMARDs).
