Combination regimen of three drugs: **umbralisib**, **obinutuzumab**, and **chlorambucil**. - **Umbralisib** is a small molecule inhibitor of phosphoinositide 3-kinase delta (PI3Kδ) and casein kinase 1 epsilon (CK1ε), designed to impair survival signaling in malignant B cells and modulate the tumor microenvironment. - **Obinutuzumab** is a glycoengineered type II anti-CD20 monoclonal antibody that targets CD20 on B cells, inducing cell death via antibody-dependent cellular cytotoxicity (ADCC), direct cytotoxicity, and phagocytosis. - **Chlorambucil** is a classic oral alkylating agent that causes DNA crosslinking, leading to cell death, particularly in rapidly dividing lymphocytes. The combination of obinutuzumab and chlorambucil is approved for first-line treatment of patients with **chronic lymphocytic leukemia (CLL)** who have comorbidities. Umbralisib is under investigation primarily in combination with ublituximab (not obinutuzumab or chlorambucil), but as of now, there are **no major clinical trials or regulatory approvals for the triple combination** of umbralisib plus obinutuzumab plus chlorambucil. Trials and standard of care regimens have featured umbralisib + ublituximab (U2) and obinutuzumab + chlorambucil, with each compared to the other in pivotal CLL studies[3][4][5][6][7][8].
