Umirolimus (also known as Biolimus or Biolimus A9) is a highly lipophilic derivative of sirolimus and belongs to the class of macrocyclic lactone immunosuppressants. It was specifically developed for local delivery via drug-eluting stents to prevent restenosis after percutaneous coronary intervention (PCI). Umirolimus acts by inhibiting the mammalian target of rapamycin (mTOR), leading to cell cycle arrest in the G1 phase and suppression of T cell and smooth muscle cell proliferation. This mechanism reduces neointimal hyperplasia and restenosis following stent implantation. The drug is proprietary to Biosensors International and has been used in various biodegradable polymer-coated or polymer-free stent platforms[1][3][5]. Clinical trials have demonstrated its efficacy in reducing adverse cardiac events compared with bare metal stents or other drug-eluting stents[4][6][8].
